An emergent theme from recent national NHSE incident reporting has been the increasing appearance of sharp popup blades being disguised within non-obvious harmful objects, such as pens.

  • One incident involved a package containing what staff thought were pens, ordered online and delivered directly to an In-Patient Unit. These were difficult to detect by staff checking mail. These items looked like ordinary pens on the website.
